@MarkMcClure yes you are right. It is also mentioned in other discussions that some parameters are ignored. However, by discretizing region as in Q 3 {0, 0} problem disappears.

problem 2 and 3 can be corrected by using DiscretizeRegion. dis = DiscretizeRegion[reg] and than mesh = ToElementMesh[dis, MaxCellMeasure -> 0.0005, "MaxBoundaryCellMeasure" -> 0.01];
